ホリナ-トカルシウムは、ジヒドロ葉酸還元酵素の結合と、葉酸からテトラヒドロ葉酸への変換を阻害する作用によって、葉酸拮抗薬の効果を相殺します。医療では、メトトレキサ-ト投与後にメトトレキサ-トの毒性を軽減させるためにホリナ-トカルシウムが使用されます。また、がん治療において、5-フルオロウラシルの細胞毒性作用の促進にも使用されます。
