シルデナフィルは、cGMP特異的ホスホジエステラーゼ5型(PDE5)に対する強力かつ選択的なインヒビターです。シルデナフィルは勃起不全や肺高血圧症の治療に使用されています。NOがグアニル酸シクラーゼを活性化すると、cGMP量が上昇して平滑筋弛緩が起こります。シルデナフィルは、cGMPの分解に関与するPDE5を阻害することにより、このNOの効果を増強します。
